NO. 1 – 18 Years and Older

$325 for the year

• 2 Professional Dental Cleanings per year
• 2 Comprehensive Doctor Exams per year
• All Needed X-rays
• 1 Emergency exam per year
• 15% off all other treatments

For patients with the absence of periodontal disease

NO. 2 – Periodontal Disease

$425 for the year

• 3 Periodontal Maintenance cleanings per year
• 2 Comprehensive Doctor Exams per year
• All Needed X-rays
• 1 Emergency exam per year
• 15% off all other treatments

For patients with stabilized periodontal disease and are actively enrolled

NO. 3 – Periodontal Disease

$525 for the year

• 4 Periodontal Maintenance cleanings per year
• 2 Comprehensive Doctor Exams per year
• All Needed X-rays
• 1 Emergency exam per year
• 15% off all other treatments

For patients with periodontal disease and are actively enrolled

Dental Office Membership Program Terms & Conditions

This Membership/Loyalty Program is not dental insurance, does not meet minimum essential coverage requirements, and is not a qualified health plan under any federal or state law.

This program is a discount and payment arrangement offered exclusively by Pomona Dental Practice of Yvonne Shu, DDS, and is not transferable to any other dental office, provider, or specialist.

Membership benefits may not be combined with dental insurance, Medi-Cal, PPO/HMO plans, government programs, third-party payers, other discounts, coupons, or promotional offers.

Membership benefits apply only to the enrolled patient and cannot be transferred, shared, sold, or assigned to another individual.

Membership benefits are valid for one (1) year from the date of purchase and do not roll over if unused.

All membership fees must be paid in full at the time of enrollment. Partial payments are not accepted unless otherwise approved in writing by the office.

All dental services are due and payable at the time services are rendered, regardless of membership status.

No refunds will be issued under any circumstances, including but not limited to failure to schedule appointments, missed appointments, early termination, relocation, or changes in dental condition.

Membership discounts apply only to services performed and completed while the membership is active. Membership discounts do not apply to services already started, completed, or paid for prior to enrollment.

Missed appointments, late cancellations, or no-shows may be subject to separate cancellation or no-show fees, which are not discounted by the membership.

Membership discounts do not apply to services provided by outside specialists, laboratories, or third-party providers.

If Care Credit is used to pay for the membership or dental services, a maximum discount of 10% will apply.

If a credit card is used to pay for dental services, a maximum discount of 10% will apply.

By enrolling, the patient acknowledges they have read, understand, and agree to all terms and conditions of the Dental Membership Program.
